<p><strong>Biomimetics</strong> is a scientifically proven set of techniques and methodologies that focus on preserving teeth and improving bonding and sealing of broken or decayed teeth in ways that mimic natural processes of teeth. This unique approach allows dentists to use small onlays that move, flex and function like actual teeth. This is in stark contrast to the large porcelain crowns currently used in traditional dentistry. Porcelain crowns restrict movement and simply do not allow teeth to function normally.</p>
<p><strong>Biomimetic dentistry</strong> utilizes minimally invasive procedures to protect and conserve a patient’s teeth. This is in contrast to conventional dentistry procedures which can compromise the structure of teeth through extensive drilling and amputation. These invasive procedures used by traditional dentistry are usually followed by incorporation of non-bonding materials used for fillings and crowns, eventually resulting in cracks, allowing bacterial infections that destroy the tooth, and the subsequent need for root canals, implants and bridges. However, up to 90% of crowns can be avoided with early detection and preventive medicine procedures utilized in biomimetic dentistry.</p>
<p>The superior materials and adhesive bonding processes used in biomimetic dentistry literally lock out bacteria, providing safer and more durable dental work. Each protective layer, using the most advanced dental materials, is applied in similar manner to the original enamel and dentin materials that comprise teeth.</p>
<p>The goal of biomimetic dentistry is to continually preserve the patient’s teeth during each dental procedure. These innovative dental procedures and proprietary adhesive bonding processes (using state-of-the-art composite resins, ceramics/porcelain, and other materials) restore teeth to their natural structure and color. Aesthetic procedures and computer-aided design (CAD) and manufacturing (CAM) are utilized to provide beautiful white uniform teeth.</p>
<p>Unlike other parts of our body, our teeth often do not mend on their own. This is why it is important to protect and conserve as much of the patient’s natural tooth and gum structure as possible. The most successful dentistry is comprised of procedures that are the least invasive, thus avoiding further damage to the patient’s teeth and gums. This is the philosophy and primary emphasis behind the origin of biomimetic dentistry and holistic approaches for keeping the patient’s teeth and gums healthy and beautiful.</p>

				<content:encoded><![CDATA[<p>The <strong>initial dental examination</strong> is the most important event in the entire oral health program for the patient.  At this time, the patient has his or her current oral status carefully assessed and a treatment plan is designed to meet your dental needs, which includes a comprehensive dental program to bring you to complete dental health.  New patient appointments last approximately 1 1/2 hours, from the welcome greeting to the exit experience here at Dental Arts Hawaii.</p>
<p>During your first visit, Dr. Baysa will review with you your medical and dental history, including:</p>
<ul>
<li>Sufficient x-rays to complete your dental exam</li>
<li>A comprehensive periodontal (gum) evaluation</li>
<li>Evaluation for tooth decay</li>
<li>Soft tissue examination of the lips, tongue and oral cavity</li>
<li>Orthodontic Screening Evaluation</li>
</ul>
<p>Based on our findings at this time, we will determine your treatment plan and schedule your next appointment to begin your dental care.  As part of your personal treatment plan, your dentist will work closely with you on a preventive program to minimize costly dental problems in the future.</p>

<p>A lot of things make us different. First, we complete a comprehensive examination of your teeth, gums, joints and other tissue in your mouth when you first visit us. We even create a digital photo album of your mouth using our digital x-ray and intra oral camera. That way, we can see everything going on in your mouth and show you what is happening as well.</p>
<p>From there, we create a customized dental treatment plan that is designed around your life. In this plan, we consider a number of factors, such as the stability of your bite and the structure of your jaw.</p>

Additional motors can be fitted to cut downtime as burrs are changed. Communication is by wireless LAN, and again the milling unit can go anywhere in the practice.</p>
<p><img src="http://www.ceramicsystems.co.uk/images/milling-units.jpg" alt="Milling Units" width="172" height="230" align="right" border="1" />Once the milling unit is started, the acquisition unit may be used for other tasks. Indeed by using the quadrant programme, you can continue designing neighboring teeth cutting time even further. The milling unit communicates with the acquisition unit via a cable or by radio. Using radio communication allows the milling unit to be installed in a wide variety of locations: indeed anywhere in the practice. The MCXL for instance being 40% quieter than the standard ML is often sited in the waiting area as a fascinating eye-catcher. The choice is yours.</p>
<p>The diamond burrs in the milling unit are designed for the shortest possible milling times. The conical burr allows creation of highly detailed occlusal surfaces. Performed symmetrically, the milling process ensures that the ceramic materials are machined with utmost care and precision.</p>

				<content:encoded><![CDATA[<h3>Prophylaxis Recommendations: Infective Endocarditis (IE)</h3>
<p>With input from the ADA, the <a href="http://www.heart.org/" target="_blank">American Heart Association (AHA)</a> released the current recommendations for the prevention of infective endocarditis in 2008.</p>
<p>The complete recommendations, including the recommended regimen, can be found in:</p>
<ul>
<li><a href="http://jada.ada.org/cgi/content/abstract/139/suppl_1/3S" target="_blank">The Journal of the American Dental Association (JADA): Prevention of Infective Endocarditis: Guidelines from the American Heart Association</a></li>
</ul>
<p>The current recommendations recommend premedication for a smaller group of patients than previous versions. This change was based on a review of scientific evidence, which showed that the risk of adverse reactions to antibiotics outweigh the benefits of prophylaxis for most patients. Concern about the development of drug-resistant bacteria also was a factor.</p>
<p>Also, the data are mixed as to whether prophylactic antibiotics taken prior to a dental procedure prevent IE. The recommendations note that people who are at risk for IE are regularly exposed to oral flora during basic daily activities such as brushing or flossing, suggesting that IE is more likely to occur as a result of these everyday activities than from a dental procedure.</p>
<h4>Patient selection</h4>
<p>The current recommendations recommend use of preventive antibiotics prior to certain dental procedures for patients with:</p>
<ul>
<li>artificial heart valves</li>
<li>a history of infective endocarditis</li>
<li>a cardiac transplant that develops a heart valve problem</li>
<li>the following congenital (present from birth) heart conditions:*
<ul>
<li>unrepaired or incompletely repaired cyanotic congenital heart disease, including those with palliative shunts and conduits</li>
<li>a completely repaired congenital heart defect with prosthetic material or device, whether placed by surgery or by catheter intervention, during the first six months after the procedure</li>
<li>any repaired congenital heart defect with residual defect at the site or adjacent to the site of a prosthetic patch or a prosthetic device</li>
</ul>
<p>* Patients should check with their cardiologist if there is any question as to whether they fall into one of these categories.</li>
</ul>
<p>Patients who took prophylactic antibiotics in the past but no longer need them include those with:</p>
<ul>
<li>mitral valve prolapse</li>
<li>rheumatic heart disease</li>
<li>bicuspid valve disease</li>
<li>calcified aortic stenosis</li>
<li>congenital (present from birth) heart conditions such as ventricular septal defect, atrial septal defect and hypertrophic cardiomyopathy</li>
</ul>
<h4>Dental procedures</h4>
<p>Prophylaxis is recommended for all dental procedures that involve manipulation of gingival tissue or the periapical region of the teeth, or perforation of the oral mucosa.</p>
<h4>Additional considerations about antibiotic prophylaxis</h4>
<p>Sometimes patients forget to premedicate prior to their appointments. The recommendation is that the antibiotic be given before the procedure. This is important because it allows the antibiotic to reach adequate blood levels. However, the recommendations to prevent infective endocarditis state:</p>
<blockquote><p>&#8220;If the dosage of antibiotic is inadvertently not administered before the procedure, the dosage may be administered up to two hours after the procedure.&#8221;</p></blockquote>
<p>Another concern that dentists have expressed involves patients who require prophylaxis but are already taking antibiotics for another condition. In these cases, the recommendations for infective endocarditis recommend that the dentist select an antibiotic from a different class than the one the patient is already taking. For example, if the patient is taking amoxicillin, the dentist should select clindamycin, azithromycin or clarithromycin for prophylaxis.</p>

<h3><a id="replace" name="replace"></a>Total Joint Replacement</h3>
<p>The American Dental Association and the American Academy of Orthopedic Surgeons (AAOS) are currently in the process of developing evidence-based clinical guidelines on the topic of antibiotic prophylaxis for patients with orthopedic implants undergoing dental procedures. The ADA and AAOS do not have a joint recommendation at this time. There are differing opinions on the need for antibiotic prophylaxis. These opinions/statements are presented below to assist the dentist in making informed decisions about the prescription of antibiotics. The ADA believes that the professional goal should be consensus among the dentist and physician, which is the expressed goal being pursued in the ongoing ADA/AAOS project.</p>
